I hereby give Protect TX Fragile Kids (PTFK) permission to use my or my child’s photograph publicly for any purpose which PTFK deems proper in the interest of internet, newspapers, television media, brochures, pamphlets, instructional material, books and clinical material, medical education, knowledge, or research. All such photographs shall be the property of Protect Texas Fragile Kids and I hereby relinquish all right, title and interest therein.
With respect to the foregoing matters, no inducements or promises have been made to us/me to secure our/my signature(s) to this release other than the intention of PTFK to use or cause to be used such photographs, films and pictures for the primary purpose of promoting and aiding Protect Texas Fragile Kids and its work.
I understand that the images may be used in print publications, online publications, presentations, websites, and social media as needed to advocate for medically fragile Texas children. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
